1. No link to report the ad? AdSense used a link or a logo that allowed you to report low-quality ads.

2. Are you sure the ad is served as part of the AdSense program? It could be a third-party ad from DoubleClick.

Yeah, the same ad appeared on my GamesfortheBrain.com, where I only include AdSense... that's how I first noticed it.

Also, I just tried again on YouTube and the ad appeared again. Hovering over the link shows "pagead2.googlesyndication.com/pag..."

It sounds weird... Once a friend told me he was viewing this kind of ads in youtube, and the problem was he had some malware hijacking the pages.

Perhaps they integrate the doubleclick ads into adsense (image) atm

This kind of deceptive advertising is old but i'm surprised to see it in AdSense
